Job Description & Requirements

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Port Carbon, Pennsylvania. This position supports patients in New Castle, PA and surrounding areas and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year.

• Collaborate to provide management and delivery of patient care plans.
• Support patients in New Castle, PA and surrounding areas.
• Use clinical educators, preceptors, and supervisors to mentor and guide as needed.
• Self‑manage time and schedule while consistently meeting critical deadlines for charting.
• Provide after‑hours care as part of a 24/7/365 team.

• RN license in the state where you will work.
• Associate degree or higher from an accredited School of Nursing.
• Two years of RN experience, with hospice experience preferred.
• Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.
• Demonstrated experience with tablets, mobile phones, and EMR software.
