Nurse Practitioner, Public Health Nurse
GENERAL OVERVIEW
In addition to base compensation, this role offers the opportunity to earn an annual quality‑based bonus tied to contributions to population health initiatives and value‑based metrics. The position forms an integral part of the care delivery system, working to provide a healing environment with a team‑based approach, improving patient outcomes and driving efficient, high‑quality care. The incumbent provides primary healthcare services for non‑acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice, within the CRNP scope of practice.
Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on‑call coverage on a rotational basis as required.
- Provide appropriate patient care including recording medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting results, and documenting accurately (25%).
- Communicate and coordinate patient care with patients, families, community health services, and interdisciplinary team; involve attending physician regarding plan of care and clinical decision making outside the CRNP's knowledge base or scope of practice (15%).
- Provide monitoring and continuity of care and coordinate periodic evaluation by physicians as defined in the written agreement between the CRNP and supervising physician(s) (10%).
- Prepare written prescription orders for drugs and controlled substances, administer medication and injections, sutures wounds, and provide wound care (10%).
- Educate patients and family members regarding health, preventive medicine, prescribed medications, treatment instructions, and recommend community resources as appropriate (10%).
- Triage patient telephone calls, provide monitoring and continuity of care (10%).
- Participate in, develop, and implement performance and/or quality improvement initiatives; ensure compliance with all regulatory agencies (10%).
- May perform other duties such as managing outpatient concerns and any other duty specific to specialty (10%).
- Other duties as assigned or requested (10%).
- Required:
Master’s degree MSN from an approved Master’s Program, National Board Certification for Nurse Practitioners via an accredited association, state licensure as a Nurse Practitioner, prescriptive authority in the practicing state, DEA licensure within 60 days of hire, CPR – American Heart Association, Act 34 Criminal Background Clearance Certificate, Act 33 Child Abuse Clearance Certificate, Act 73 FBI Fingerprinting Criminal Background Clearance Certificate. - Preferred:
Master’s degree in Physician Assistant Studies, current PA‑C license or board eligible, ongoing successful recertification (NCCPA) as a PA‑C, DEA licensure within 60 days of hire, CPR – American Heart Association, Act 33 Child Abuse Clearance Certificate, Act 34 Criminal Background Clearance Certificate, Act 73 FBI Fingerprinting Criminal Background Clearance Certificate. - Substitutions:
Four to ten years related experience and/or training; or an equivalent combination of education and experience.
