We are seeking an experienced Licensed Practical Nurse (LPN) to join our Home Care team. The ideal candidate will have at least 2 years of home care experience and strong expertise in wound care. This role involves providing skilled nursing care to patients in their homes while promoting recovery, independence, and overall well-being.
Responsibilities- Provide skilled nursing care to patients in their homes.
- Perform patient assessments and monitor changes in condition.
- Deliver advanced wound care, including wound assessment, dressing changes, and infection prevention.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on wound management, medications, and disease management.
- Maintain accurate and timely clinical documentation.
- Collaborate with physicians, case managers, therapists, and other healthcare professionals to ensure quality patient care.
- Follow agency policies, state regulations, and infection control standards.
- Active Pennsylvania LPN License.
- Minimum 2 years of Home Care experience.
- Strong wound care experience is required.
- Excellent assessment, communication, and documentation skills.
- Valid driver's license and reliable transportation (if required by the employer).
- Current BLS/CPR certification preferred.
- Coverage Area: West Philadelphia, PA (ZIP Codes 19104 & 19139).
- Candidates must have wound care proficiency to be considered.
- Home care experience is required.
