Position Overview

Registered Nurse (RN) – Home Health;
Natrona Heights, Pennsylvania.

• Deliver skilled, patient‑centered care in a home health setting.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management.
• Think critically, solve problems in real time, and document thoroughly and compliantly in the EMR.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within the assigned territory.

• Active Registered Nurse license in the state of employment.
• Minimum one year of RN experience providing quality patient care.
• Current driver’s license and ability to drive approximately 20% of the day to patient locations.

• Base pay with a $10,000 first‑year success bonus and ongoing bonus potential.
