Responsibilities
Oversee population to ensure timely progression of care, safe discharge planning, and appropriate post‑acute services using evidence‑based criteria. Identify patient clinical conditions, testing and value‑based clinical interventions that can be completed in the ambulatory space with appropriate follow‑up. Apply clinical criteria to monitor medical necessity, reduce avoidable days, and resolve barriers to care in collaboration with physicians, nursing, and utilization management.
Collaborate in the development and implementation of performance improvement activities with care management, physicians, and utilization management. Interact with co‑workers, visitors, and other staff consistent with the values of Jefferson. Monitor length of stay, readmissions, and throughput metrics; identify trends; and contribute to initiatives that improve efficiency and patient outcomes.
- Bachelor’s degree in Nursing
- 5 years of inpatient acute care hospital experience
- Excellent communication (oral and written) to facilitate reviews and discussions with providers
- Registered Nurse (RN) licensed in Pennsylvania or New Jersey upon hire
- Master’s degree in Nursing (preferred)
- Knowledge of care and utilization management (preferred)
- Strong interpersonal and organizational skills to manage case load and communicate with multi‑disciplinary team members (preferred)
- Ability to work independently and set priorities to ensure appropriate use of hospital resources (preferred)
- Ability to lead individuals and groups (preferred)
Lifts and carries 40‑50 lbs. Push/pull patients on bed or stretcher (requires 29 lbs. push force); lateral transfers up to 50 lbs. of the patient’s weight. Frequent to continuous standing or walking. Patient transporters can walk 8‑10 miles per shift. May be required to kneel, bend, squat, and perform CPR.
