Job SummaryThe LPN Float position is a flexible, field-based role that supports both home care and home health clients. This role requires travel within and outside of Philadelphia to deliver high-quality nursing care while adhering to all clinical and documentation standards.
Key Responsibilities- Provide skilled nursing care to home care and home health clients, including wound care, medication administration, monitoring patient conditions, and patient education.
- Collaborate with the care team to ensure coordinated and effective patient care.
- Respond promptly to changes in patient conditions and communicate updates to the care team.
- Complete all patient visit documentation accurately and submit it within 24 hours of the visit.
- Maintain accurate, up-to-date records in compliance with agency policies and regulatory requirements.
- Accept all assigned visits and complete them in a timely manner.
- Adhere to all federal, state, and agency regulations and standards.
- Attend required training sessions and team meetings as needed.
- Valid LPN license in the state of Pennsylvania.
- Minimum of 1 year of nursing experience (home care or home health preferred).
- Strong organizational and time-management skills.
- Ability to work independently and handle a diverse caseload.
