About the Role:
As an Evening Shift LPN at Haven Place, you'll be responsible for providing direct patient care, including:
Administering medications and treatments as ordered by physicians
Assisting with activities of daily living, such as bathing, dressing, and grooming
Monitoring vital signs and reporting changes to the healthcare team
Providing emotional support and companionship to our residents
Collaborating with the interdisciplinary care team to develop and implement individualized care plans
Responsibilities:
Provide direct patient care under the supervision of RNs and other healthcare professionals
Conduct vitals checks, lab tests, and other medical procedures as needed
Document patient information accurately and thoroughly
Participate in quality improvement initiatives to enhance patient outcomes
Maintain a safe and organized work environment, adhering to all infection control guidelines

Requirements:
Current LPN license in the state of Pennsylvania
