Home Health Registered Nurse (RN) - Monday-Friday | No Weekends/On-Call

We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality patient care across Snyder, Union, and Northumberland Counties. This role offers a Monday-Friday schedule, no weekend or on-call requirements, and mileage reimbursement between patient visits. Productivity expectation is 30 points per week.

Pay Rate: $50/hr

• Provide Skilled Nursing Care — Conduct assessments, administer medications, perform wound care, and deliver patient/caregiver education in the home setting.
• Develop and Manage Care Plans — Create individualized care plans based on physician orders and patient needs.
• Collaborate with Interdisciplinary Team — Communicate with physicians, therapists, and social workers to ensure coordinated care.
• Complete Accurate Documentation — Maintain timely and compliant electronic medical records.
• Monitor Patient Progress — Identify changes in condition and report promptly to the care team.
• Educate Patients and Families — Teach disease management, medication adherence, and safety practices.
• Ensure Regulatory Compliance — Follow all state, federal, and agency guidelines.

• Active PA RN License — Verify PA RN licensure
