Triage LPN

Job Type: Full-Time

Position Summary
The Triage LPN provides telephone and clinical triage to evaluate patient needs, coordinate care, and ensure timely communication with physicians and the healthcare team. This role requires strong clinical judgment, communication, and the ability to remain calm and organized in a fast-paced environment.

Key Responsibilities

  • Conduct telephone assessments and obtain accurate patient medical histories.
  • Evaluate patient symptoms and determine appropriate next steps based on established protocols.
  • Communicate with physicians and other healthcare professionals regarding patient needs.
  • Respond to calls and tasks related to test results, medication refills, post-operative instructions, and general medical questions.
  • Handle complicated prior authorizations and medical clearance requests.
  • Initiate appropriate diagnostic or therapeutic measures according to standing protocols.
  • Coordinate patient needs with other departments and assist with resolving patient care concerns.
  • Provide clear patient education and instructions regarding medications, treatments, and follow-up care.
  • Maintain accurate and timely documentation in the EMR.
  • Protect patient confidentiality and comply with HIPAA and all organizational policies and regulations.
  • Provide compassionate, professional service to patients and families.
  • Perform other clinical duties as assigned.

Requirements

  • Graduate of an accredited LPN program.
  • Current LPN license as required by the state.
  • BLS certification required; ACLS preferred.
  • 3+ years of related medical/clinical experience preferred.
  • Triage experience preferred.
  • Strong telephone triage and clinical assessment skills.
  • Excellent communication, organization, critical thinking, and problem-solving abilities.
  • Comfortable working independently and making sound clinical decisions in a fast-paced environment.
  • Proficiency with EMR systems and basic Microsoft Office applications.
