Registered Nurse – Home Health

Home Health Nursing Position

Provide comprehensive home health nursing care to patients in their residence, ensuring optimal health outcomes and patient comfort. Collaborate with interdisciplinary teams to deliver personalized treatment plans and promote independence.

Responsibilities:

• Assess patient conditions and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and document clinical findings accurately.
• Educate patients and caregivers on health management and disease prevention.
• Coordinate with healthcare team members to ensure continuous, high-quality care.
• Comply with all safety, privacy, and healthcare regulations and protocols.

Qualifications:

• Hold a valid registered nurse (RN) license in the state of practice.
• Demonstrate at least 1-2 years of experience in home health nursing or relevant setting.
• Possess strong assessment, communication, and patient education skills.
• Be proficient in electronic health records (EHR) and documentation.
• Maintain current CPR and BLS certifications.
• Exhibit compassionate, patient-centered care approach.
