• Develops, implements, and revises care plans in collaboration with physicians and team members.
• Supervises home health aides and nurses, provides instruction, and ensures adherence to regulations.
• Performs comprehensive assessments, medication reconciliation, and accurate documentation of patient visits.
• Ensures patient eligibility, communicates effectively with patients, families, and healthcare team, and participates in quality improvement and performance activities.
• Supports team orientation, participates in survey readiness, and contributes to reducing hospitalizations and enhancing patient safety.

• Current and unrestricted RN license in the state of practice.
• Valid driver's license, vehicle insurance, and reliable transportation.
• Preferred: CPR certification, 1+ years of home health experience, strong communication, organization, and independence skills.
