As an LPN Licensed Practical Nurse, you will play a vital role in our Weekend Program, providing high-quality care to our patients in a fast-paced and dynamic environment. Your responsibilities will include:

• Providing direct patient care, including administering medications, dressing wounds, and performing vital signs
• Developing and implementing individualized care plans in collaboration with our interdisciplinary team
• Maintaining accurate and detailed records of patient care and treatment
• Collaborating with other healthcare professionals to ensure seamless care transitions and communication

• Deliver exceptional patient care, prioritizing individual needs and preferences
• Work closely with colleagues to ensure a smooth and efficient care delivery
• Stay up-to-date with the latest developments in patient care and evidence-based practices
• Participate in quality improvement initiatives to continually improve patient outcomes

• 1 year of experience as an LPN Licensed Practical Nurse
• Current LPN License
• Strong communication and interpersonal skills
