Registered Nurse

Laurel Center - PA and V2U seek a compassionate Registered Nurse to provide high-quality, resident-centered care to seniors in our long-term care and rehabilitation community. The RN will perform comprehensive assessments, administer medications and treatments, and coordinate individualized care plans in collaboration with an interdisciplinary team. Responsibilities include monitoring resident conditions, accurate EHR documentation, supporting rehab and chronic care goals, and guiding support staff.

Responsibilities

• Deliver direct nursing care to senior residents, including assessments and treatments
• Administer medications and therapies per provider orders and facility protocols
• Develop, implement, and update individualized care plans
• Monitor resident status, document in EHR, and report changes promptly
• Collaborate with physicians, therapists, and care teams to support rehab and long-term care goals
• Educate residents and families on care plans and health management
• Support safe infection control and quality, regulatory-compliant practices
• Supervise and support LPNs and CNAs to ensure high-quality care
