Role: Registered Nurse in Home Health, providing and coordinating nursing care in patients' homes.
Key Responsibilities- Develop and update patient care plans collaboratively with physicians and the healthcare team.
- Perform comprehensive nursing assessments, including OASIS and medication reconciliation.
- Provide high-quality clinical care, supervise and assign tasks to aides and LPNs/LVNs, ensuring compliance with regulations.
- Accurately document patient care, communicate effectively with physicians, patients, families, and staff.
- Participate in staff orientation, performance improvement, safety initiatives, and efforts to reduce hospitalizations.
- Maintain professionalism, punctuality, ongoing learning, and participate in on-call/weekend rotations.
- Adapt to diverse home environments regardless of client background.
- Current RN licensure in the practicing state.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
- Preferred: CPR certification (or ability to obtain within 90 days) and 1+ years of clinical experience.
