Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and be skilled in various nursing practices. As a Charge Nurse, you will play a vital role in providing high-quality care to patients in diverse settings, including home care and long-term care facilities. Your expertise will contribute significantly to the health and well-being of our patients.
Duties Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications and treatments as prescribed by physicians.
Perform procedures needed per physician orders
Monitor patient progress and respond to changes in condition.
Collect and process specimens for laboratory testing.
Provide education and support to patients and their families regarding health management.
Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
Document all patient interactions accurately in electronic health records (EHR) systems.
Participate in ongoing professional development and training.
