• Provide skilled nursing care in patients’ homes, including assessment, medication administration, wound care, and symptom management.
• Perform start-of-care (SOC) and ongoing reassessments, developing and updating individualized care plans with the care team.
• Conduct and document home health visits per plan of care, monitor patient status and safety indicators, and elevate changes promptly.
• Educate patients and families on disease management, medications, safety at home, and self-management strategies to support adherence.
• Maintain accurate documentation in the electronic chart, ensure regulatory compliance, and coordinate with the interdisciplinary team to optimize outcomes.

• Active Pennsylvania RN license in good standing
• 2+ years of home health nursing experience
• Start of Care (SOC) and revisit experience required
• RN degree (Associate or Bachelor’s) from an accredited program
• Ability and willingness to travel to patient homes within service ZIP codes: 19040, 19044, 18929, 18974, 18976, 19002, 18954, 18966, 18901, and to work in a home health setting
