Responsibilities:- Provide direct patient care to residents on the evening shift, including administering medications, wound care, and other nursing interventions
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Monitor and report resident conditions, progress, and concerns to the nursing staff and other healthcare professionals
- Collaborate with other healthcare professionals to ensure continuity of care and effective communication
- Participate in quality improvement initiatives to enhance the overall quality of care and services
- Maintain accurate and thorough records of patient care and experiences

Requirements:- Valid LPN (Licensed Practical Nurse) license in the state of Pennsylvania
- Current CPR certification
- Experience in a long-term care or rehabilitation setting (not required but desirable)
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment with attention to detail and prioritization skills
