Social Services Director

Location: Myerstown, PA

We are seeking a dedicated Social Services Director to join our skilled nursing team in Myerstown, PA.

The Social Services Director serves as a liaison and advocate for residents' social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department.

Job Functions
  • Develop comprehensive social assessments and care plans identifying residents' medically related social and emotional needs with realistic goals and interventions.
  • Actively participate in pre-admission and admission processes.
  • Provide compassionate support to residents and families during adjustment, crisis, or special circumstances.
  • Maintain accurate documentation of residents' behavioral and psychosocial status.
  • Provide individual and group support services for residents and families.
  • Establish and grow a volunteer program to meet social service needs.
  • Orient residents and families to the facility and assist with transition and adjustment.
Qualifications
  • Bachelor's degree from an accredited school in Social Work or a related field required.
  • Strong knowledge of social service practices, procedures, laws, and regulations in long-term care.
  • Ability to work independently with strong interpersonal and communication skills.
