Position: County Caseworker 2 (LG) Supports Coordinator - Wayne County MH/ID
Location: Bensalem

Located in Honesdale, Pennsylvania, Wayne County is seeking a compassionate, community‑minded professional to join our team as a Supports Coordinator.

Responsibilities
  • Service Planning: develop Individual Support Plans and review them regularly to match each person’s needs.
  • Support Coordination: connect individuals with natural supports and necessary services based on their goals.
  • Community Engagement: encourage participation in community activities and employment when appropriate.
  • Field Visits: travel to meet individuals and teams in person to review progress and needs.
  • Record Management: complete documentation and data entry to maintain accurate and timely records.

    work closely with families, supervisors, and peers to support each individual’s growth.
Work Schedule & Additional Information
  • Full‑time employment.
  • Work hours are 8:00 AM to 4:30 PM, Monday‑Friday, with a 1‑hour lunch.
  • Occasional overnight and out‑of‑county travel for meetings or training.
  • Coordinators are not on call, though may be called out in an emergency.
  • Telework: this position does not offer telework.
Qualifications
  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor’s degree with a social welfare major; or
  • A bachelor’s degree supplemented with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training including 12 college credits in the above fields.
  • Applicants within two months of graduation will be considered to have met the educational requirements.
Other Requirements
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.
Legal Requirements
  • A conditional offer of employment will require a drug screening.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the law, a conditional offer will require submission and approval of satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
