Supports Coordinator
Located in Honesdale, Pennsylvania, Wayne County wants you to make a difference every day! We are seeking a compassionate, community-minded professional to join our team as a Supports Coordinator. This position supports individuals with intellectual and developmental disabilities by helping them access services and stay connected to their communities. You will work with families, teams, and providers to ensure each person has what they need to meet their goals. As a Supports Coordinator, you will perform the following duties:
- Service Planning: Develop Individual Support Plans and review them regularly to match each person's needs
- Support Coordination: Connect individuals with natural supports and necessary services based on their goals
- Community Engagement: Encourage participation in community activities and employment when appropriate
- Field Visits: Travel to meet individuals and teams in person to review progress and needs
- Record Management: Complete documentation and data entry to maintain accurate and timely records
- Team Collaboration: Work closely with families, supervisors, and peers to support each individual's growth
Work Schedule and Additional Information:
- Full-time employment.
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch.
- Occasional overnight and out of county travel for meetings or training.
- Coordinators are not on call, however may be called out in an emergency.
- Telework: You will not have the option to telework in this position.

Minimum Experience and Training Requirements:
- Six months of experience as a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- A bachelor's degree with a social welfare major; or
- A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
- Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
Other Requirements:
- PA residency requirement is currently waived for this title.
- You must be able to perform essential job functions.
Legal Requirements:
- A conditional offer of employment will require a drug screening.
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
