Richboro Rehab and Nursing in beautiful Richboro, PA is hiring a Full time Social Services Director.

• The Social Services Director collaborates with the interdisciplinary team to promote and protect resident rights and the well‑being of all patients/residents. This includes educating staff on the role of the social services
• The Social Services Director is a leader on the interdisciplinary team for the written care plan for each residents social service’s needs.
• The Social Services Director holds meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life.
• The Social Services Director performs patient/resident discharge planning activities such as family/patient education, nursing center documentation, and make referrals to appropriate agencies and suppliers.
• The Social Services Director establishes relationships and maintain contact and referral flow with community based agencies/services.

• Bachelor's degree from accredited school of Social Work or related field is required.
• At least one year of supervised social work experience in a Skilled Nursing Facility, working directly with individuals, is preferred.
• Licensed in Social Work for the state of PA.
