- Housing and Urban Development / Veterans Administration Supportive Housing (HUD/VASH) position provides outreach, assessments, referrals and case management services to homeless veterans identified in the community. Services are provided within outpatient clinic, community, home and transitional housing locations. The incumbent has expectations to meet the Veteran in the community to provide services with use of mobile technology, including employee safety devices.
The employee provides a full range of social work services within commonly accepted standards of social work practice, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. S/he provides ongoing therapeutic intervention with the veteran patient and significant other to assist them in dealing and coping with the psychosocial issues of mental health, aging, illness and disability, dealing with loss and grief.

Duties and Responsibilities:
- Serves as case manager for identified caseload within HUD/VASH Program within designated community coverage locations, completes in person and in home visits, conducting outreach within multiple county catchment areas at day shelters, soup kitchens or other locations serving homeless Veterans
- Conducts patient intake assessments, referrals, treatment plans, case management notes, consults, and discharge documentation in a timely manner within the medical records standards with Computerized Patient Record System (CPRS) and Homeless Operation Management and Evaluation system (HOMES)
- Provides phase case management services to reflect housing first model towards self-sufficiency for housing with required in-home service delivery visits
- Provides direct assistance as indicated for Public Housing Choice Voucher applications, rental searches, grant applications, and other relevant services
- Provides referral management assistance and monitoring for community programs, VA Health Care services, public welfare program
- Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds
- Provides clinical social work assessment, intervention, treatment and follow-up, using a social determinant of health framework
- Develops understanding of VA Health Care eligibility and related eligilbity for VA Homeless Programs
- Participates with Community Homeless Regional Housing Advisory Boards for Veterans by Name list and service coordination.
- Assists with navigation between varying levels of care and conducts comprehensive transition planning
- Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services

Pay:
Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
