Position
Blended Case Manager – Salisbury Behavioral Health, RHA Health Services. Full‑time, regular position.
OverviewBlended Case Management (BCM) assists children and adults with intellectual, developmental disabilities, mental health, and substance use challenges by building a strong network of community resources and services. The BCM program links clients to needed services in the community and provides 24/7 crisis intervention on a rotational basis.
Responsibilities- Assist consumers in gaining access to public and private resources/services (medical, vocational, educational, employment, etc.) as specified in the service plan.
- Communicate with consumers weekly to determine progress and revise the service plan.
- Resolve consumer problems and concerns.
- Develop and enhance consumer social support networks with families, schools, workplaces, and neighborhood settings.
- Observe and respond to significant changes in mental and physical health and communicate changes to the treatment team.
- Collaborate with county Office of Mental Health personnel and other service providers to assess needs and provide appropriate services.
- Provide 24/7 on‑call crisis services on a rotational basis.
- Maintain case records and complete documentation, letters, assessments, and summaries as required by policy.
- Approximately 75% of work hours are out of the office in the community.
- Bachelor’s degree in a human services related field (sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education) OR an Associates degree with 12 college credits in a relevant field and two (2) years’ experience in mental health/human services.
- Excellent communication, writing, and time‑management skills.
- Ability to establish and maintain effective relationships with consumers, families, and staff from varied cultures and backgrounds.
- Valid Pennsylvania driver’s license with a clean record.
- Ability to pass background checks and screenings.
- Pay: $20.00/hr.
- Sign‑on bonus: $1,000 paid in three increments ($250 after 90‑day probation, $250 after 6 months, $500 after 12 months).
