The Family Preservation and Prevention Ongoing Services (FPPOS)-Case Manager provides ongoing, family-centered case management to children, youth, and caregivers. The position monitors child safety and family stability, supports progress toward service plan goals, coordinates referrals, and maintains timely communication and documentation in accordance with VQ, county and state requirements.
- Case Management and Family Engagement:
- Engage children, youth, and caregivers using trauma-informed, strengths-based, and family-centered practices.
- Complete ongoing assessments of family needs, strengths, safety concerns, protective factors, and barriers to service participation.
- Develop, implement, and monitor individualized service plans in collaboration with families, the referring agency, and involved providers.
- Conduct required monthly in-home visits with all children and maintain additional contact based on family need, risk, and agency direction.
- Monitor child safety, family stability, service participation, and progress toward established goals; promptly report emerging concerns through required channels.
- Assist families with accessing behavioral health, medical, educational, substance use, parenting, housing, employment, transportation, and other community supports.
- Provide practical support, coaching, and resource navigation to strengthen caregiver capacity and family functioning.

Collaboration: - Maintain regular communication with referring agency caseworkers and supervisors regarding family progress, risks, service needs, and barriers.
- Coordinate with schools, behavioral health providers, medical professionals, courts, juvenile probation, community organizations, and other involved systems.
- Participate in agency staffings, multidisciplinary case reviews, supervisory meetings, court proceedings, and other required meetings.
Coordinate and track referrals, follow up on service access and participation, and support continuity of care. - Prioritize evidence-based, Family First-aligned, and community-based services when appropriate and available.
- Documentation, Reporting, and Compliance:
- Complete case notes, assessments, service plans, contact records, court reports, weekly reports, statistical reports, and other required documentation accurately and within established timelines.
