This position focuses on delivering specialized care management and protective services to older adults and their families. The role ensures that clients receive proper support, advocacy, and connection to needed resources. As an Aging Care Manager 3, you will perform the following duties:
- Specialized Care: Provide care management services that support older adults and improve their quality of life
- Protective Services: Assist older adults who face abuse, neglect, exploitation, or abandonment
- Community Coordination: Serve as a liaison to social service agencies and community resources to meet client needs
- Professional Consultation: Consult with staff in other program areas regarding assessments, diagnoses, and treatment plans
- Client Stabilization: Develop plans that promote safe living situations or improve daily living conditions
- Supervisory Duties: Perform supervisory responsibilities and participate in subordinate work as needed
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
- Telework: You will not have the option to telework in this position.

Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

- Hiring preference for this vacancy may be given to candidates who live within Northumberland County. If no eligible candidates who live within Northumberland County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
