RHA Health Services, LLC is hiring a Social Worker / Case Manager to help individuals with intellectual and developmental disabilities, mental health, and substance use challenges live independently in the community. You will develop and implement individualized plans and coordinate services with hospitals, clinics, and social service agencies.
The role requires a bachelor’s in social work (MSW preferred), at least one year of relevant experience, and a valid driver’s license.
