• Join a Mission-Driven Organization What You’ll Do The Case Manager provides community-based support and coordination of services for program participants. This role is responsible for assessing participant needs, connecting individuals with community resources, and supporting the implementation of individualized recovery or service plans. The Case Manager works collaboratively with multidisciplinary teams to promote recovery, stability, and independence for participants. Your responsibilities may include:
Coordinate and implement individualized service and recovery plans. Provide community-based case management services and support participants in achieving personal goals. Assist participants with accessing resources such as housing, medical care, employment, education, and behavioral health services. Advocate on behalf of participants to ensure access to necessary supports and services. Transport participants to appointments and community resources when required. Collaborate with clinical staff, community providers, and other stakeholders.
Maintain accurate and timely documentation in accordance with program and regulatory requirements. Identify service gaps and communicate recommendations for improving participant services. Attend team meetings, supervision, and required trainings.
* Ensures accurate billing and statistical reporting for clients on caseload, and → monitors billing/statistical accuracy across the case management team, flagging discrepancies before submission.
* Ensures communication of pertinent information to outside agencies to assist in providing basic needs for clients, such as IEPs, prior drug abuse or mental health history.
* Completes client satisfaction index at time of discharge.
* Completes referrals and intake processes, and → provides guidance to case management staff on complex or time-sensitive intake/referral situations. Assists with client behaviors and unit operations as needed. Provides coverage for the Program Manager or APM on case-management-related matters in their absence, as assigned.
