Blended Case Manager

Children Service Center seeks a Blended Case Manager to support children, adolescents, and families with behavioral health needs in Wilkes-Barre, PA. In this role, you will provide trauma-informed case management, including assessment, service planning, care coordination, and linkage to community resources across outpatient, school-based, residential, and crisis programs. Working within a collaborative, mission-driven team, you will advocate for clients, engage families, and maintain thorough documentation while helping to improve mental health and family stability in the community.

Responsibilities

• Provide blended case management services to children, adolescents, and families with behavioral health needs
• Conduct intake, assessment, and person-centered service planning
• Coordinate care across outpatient, school-based, residential, and crisis services
• Link clients to community resources, supports, and benefits
• Monitor progress, adjust service plans, and maintain accurate documentation
• Collaborate with clinicians, schools, and community partners in a trauma-informed manner
• Advocate for client needs and support family engagement
• Participate in team meetings, trainings, and quality improvement initiatives
