Qualifications
The ideal candidate will possess the following:
- Bachelor's degree from an accredited college or university (required)
- Minimum of one year of related experience
- Knowledge of child development (preferred)
- Valid driver's license and clean driving record
Position Details
The Early Intervention Service Coordinator will:
Maintain a caseload of 50+ families with minimum monthly contactServe as a liaison with Bucks County MH/DP, Behavioral Health, Children & Youth, referral agencies, advocacy groups, and stakeholder service providersEnsure program compliance with all internal, state, and county policies, regulations, and requirementsComplete intakes of Bucks County policies and proceduresFacilitate and participate in assessments, evaluations, and the development, implementation, and review of IFSPs based on program regulations and family needsAssess child needs and family concernsGather and document pertinent child development informationCoordinate the implementation of intervention servicesEducate families on outcomes and family-training, routines-based models of interventionAssist families in identifying appropriate service providers and facilitate communication between providers and familiesParticipate in the Local Interagency Coordinating Council (LICC)Maintain all mandated family contacts and case management records in compliance with Bucks County MH/DP and PA state policiesRefer clients to BCIU #22 for transition services when appropriateAttend and participate in transition planning meetings with BCIU #22Ensure documentation is professional, accurate, and submitted on timeInput, update, and maintain records in the HCSIS Pelican systemCoordinate scheduling of IFSP/team meetings with families, therapists, and agenciesObtain medical necessity forms from physicians when requiredResearch and share appropriate community activities and resources with familiesAttend all required internal/external meetings, including at least 24 hours of training per yearApply now and join a team that's truly making a difference!
