K-12 Behavioral Health Service Coordinator

The K-12 Integrated Behavioral Health Services Coordinator will provide services in grades K-12 to enhance the children's emotional well-being and improve their academic performance. The K-12 Behavioral Health Services Coordinator can be called on to help students, families, and teachers address concerns such as truancy, social withdrawal, challenging behaviors, and the effects of special physical, emotional, or economic situations. The K-12 Integrated Behavioral Health Services Coordinator often addresses issues such as substance abuse and maturation considerations in the higher-grade levels.

Through site coordination and case management, the K-12 Integrated Behavioral Health Services Coordinator will connect the school and students to provide:

• Targeted services typically provided in a group setting to students with a common need.
• Intensive, individualized services typically provided in a one-to-one setting to students with highly specific needs.
• Recruiting and onboarding processes, while overseeing and collaborating with social work interns as applicable to ensure continuous programs for students and a consistent District philosophical approach in relation to program support(s).

Responsibilities:

• Engage in social casework with targeted students.
• Coordinate with all buildings to ensure a K-12 systematic approach and support for students.
• Facilitate referrals for families to community-based services in consultation with school staff.
• Facilitate support groups in coordination with school counselors and other community providers.
• Establish support groups based on the identified needs from the Needs Assessment to support students.
• In collaboration with guidance counselors and building principals; work 1:1 with students that need specific support.
• Assist with behavioral assessments and intervention planning.
• Work collaboratively with the school principals and identified leadership to conduct a school wide needs assessment.
• Assess and develop a School Support Plan to address identified needs.
• Work with school leadership and staff to establish a referral process for identifying case managed students.
• Collaborate with staff regarding suspected child abuse/neglect.
• Participate, as directed, in special education meetings and processes, and offer assistance to parents when close cooperation between home and school is required.
• Collaborate with the District's Homeless Liaison and provide appropriate services for homeless students.
• Provide support services to at-risk students, including, but not limited to: affective, social, and coping skills.
• Support school wide and/or district wide initiatives.
• Coordinate food and resources drive and programs.
• Recruit, onboard, oversee and collaborate with social interns as applicable to ensure a continuity of programs for students and a consistent District philosophical approach in relation to program support(s).
• Make home visits, when appropriate, to advance understanding of the students' needs and to facilitate communication between home and school.
• Conduct home "well visits" as necessary to ensure safety and support for students in the home environment.
• Develop and run parent, family, and community programs during the day and after school to help build and inform community interactions.
• Provide case management services for families in need of community agency services.
• Provide services to students over the summer to ensure continuity of support.
• Link parents with appropriate resources as indicated by a needs assessment and administration/staff input.
• Promote parental involvement in school activities, conferences, and events.
• Assist the District in helping families and students comply with attendance laws.
• Support Attendance Behavior Improvement plans.
• Assist Principals with the creation of the Truancy Elimination Plans, in collaboration with parents and school staff, to help improve student attendance.
• Assist the District in helping families and students complete free and reduced lunch applications.
• Serve as a liaison with community and social service agencies, including, but not limited to:
  Child and Youth Agency, Juvenile Probation, Mental Health, Behavioral Health, Drug and Alcohol Commission, and private practitioners.
• Serve on school and community-based interagency teams and task forces relevant to the needs of students, families, and the District.
