Job Title: Home Health RN
Location: Pensacola FL 32514
Coverage Area: Escambia, Santa Rosa, Okaloosa, and Walton
Shift: M-F 8am-5pm (Some weekend availability would be great - 1 weekend per month/1 on call night per week)
Pay: $54.00 - $56.00 per hour
Scheduled Pay: Weekly
Job Type: Full time

Home Care Registered Nurse (RN) Responsibilities- Escambia, Santa Rosa, Okaloosa, and Walton
- 30 units per week / 4-6 patients/day / 20-25 patients a week
- Conducts comprehensive assessments of patient health and living conditions to develop personalized care plans and update them based on patient needs and progress.
- Administers medications, wound care, injections, and other skilled nursing services following physician orders, maintaining strict adherence to safety and infection control standards.
- Monitors patient conditions and vital signs, identifies signs of potential complications, and collaborates with primary care providers to adjust treatment plans as necessary.
- Educates patients and families on managing health conditions, post-treatment care, and medication regimens, empowering them to take an active role in recovery and ongoing health maintenance.
- Ensures accurate documentation of all patient care activities, updates electronic health records (EHR), and provides timely and detailed reports to the care team.
- Coordinates with physical therapists, occupational therapists, and social workers to ensure holistic care and support tailored to patient needs.
- Performs regular follow-up visits to monitor progress, address concerns, and modify care plans to meet evolving health and safety requirements.
- Active RN Florida State License
- Valid driver's license
- 1 year Home Health experience required
- OASIS experience required
- Homecare Homebase required
- Wound care, PICC/IV care, lab collection, drain care, Otomy care, and experience with post-surgical care is helpful
