: Escambia/Santa Rosa or Okaloosa/Walton)
$37.30/hr.
Mileage Reimbursement$0.49/mile
ScheduleMonday to Friday,8:00am-5:00pm,40 hours/wk.
On-Call
:1 day per week
We’re hiring an LPN to provide in-home nursing care while maintaining each client’s dignity, safety, and independence. This is an onsite, field-based home health role supporting patients across assigned counties in the Pensacola area.
Key Responsibilities- Provide skilled nursing care per the client’s plan of care and physician orders.
- Monitor vital signs and report abnormal findings promptly.
- Administer medications and treatments as ordered (including oxygen support, feedings, ostomy care when applicable).
- Perform wound care, assessments, and routine skilled nursing visits.
- Coordinate and follow care routines established by PT, OT, and Speech Therapy.
- Maintain infection control practices and report potential infections/communicable diseases.
- Document all care provided, patient progress, and outcomes accurately and timely.
- Order supplies/medications as needed to ensure appropriate stock on hand.
- Monitor incidents, assess the patient after incidents, and complete required incident reporting.
