Kindred at Home is seeking a Home Health LPN to provide skilled nursing services under the direction of an RN, in patients' homes. You will deliver care for illness, rehabilitation and prevention, following the plan of care and state regulations.
You will communicate changes to the RN supervisor and participate in discharge planning as appropriate. The role requires an LPN/LVN license, CPR certification, and at least two years of clinical experience, preferably in home health or hospice.
