- Support medical foster care families and improve outcomes for medically complex children and youth
- Report to the Senior Registered Nursing Supervisor
- Conduct home visits, caregiver evaluations, and child/youth care assessments
- Coordinate with caregivers, medical teams, community stakeholders, Medicaid plans, and community partners
- Advocate for safe, supportive, family-centered homes
- Provide onboarding, ongoing education, training, and support to caregivers and families
- Coordinate daily program operations, admissions, and clinical support
- Monitor documentation, services, and caregiver performance for quality assurance
- Provide after-hours guidance when urgent needs arise
- Participate in compensated after-hours on-call coverage
- Travel across Escambia, Santa Rosa, Okaloosa, and Walton counties, with approximately 20% travel
- Must be licensed in the State of Florida as a Registered Nurse in accordance with Florida Statute 464, or have a Compact License from a participating state
- Must hold a valid unrestricted Florida Driver License
- Willingness to make home and community visits and perform independent assessments of children, youth, and medical foster families
- Knowledge of nursing care for children with special health care needs
- Knowledge of the foster care system and child/family community resources
- Nursing assessment and clinical nursing skills
- Ability to provide training and education to health care professionals, parents, and medical foster care parents
- Nursing documentation skills
- Effective verbal and written communication
- Ability to plan, organize, and coordinate work assignments
- Ability to access and utilize community resources
- Ability to use computer data systems and websites
- Ability to learn and communicate effectively in English
- Pediatric nursing experience preferred
- 3–5 years of nursing experience preferred
- Experience with child welfare, Medicaid Managed Care, or care coordination/case management preferred
- Experience with electronic health records and data entry preferred
- Bachelor of Science in Nursing or higher preferred
Demonstrates expertise in nursing care for medically complex children and youth, with strong skills in assessment, documentation, and training. Proven ability to coordinate with caregivers, medical teams, and community resources while advocating for family-centered care.
