Position: Care Coordinator (SEN Program)

CDAC Behavioral Healthcare is seeking a compassionate, experienced Care Coordinator to join our SEN Team serving Circuit 1, which includes Escambia, Santa Rosa, Okaloosa, and Walton counties. The SEN program provides comprehensive care coordination and recovery support to parents experiencing substance use concerns, with the goal of promoting family stability, long‑term recovery, and healthy outcomes for children.

Position Requirements

This is not a remote position. The successful candidate must be able to travel throughout Circuit 1 and provide transportation to appointments for families when needed. Reliable personal transportation is required.

The ideal candidate will have:

  • A minimum of two years of experience working with individuals experiencing substance use and co‑occurring mental health disorders.
  • Experience supporting parents and families, particularly those involved with child welfare or facing complex family challenges.
  • Knowledge of community resources and the ability to connect families with appropriate services.
  • A strong understanding of the impact of substance use on parents and young children, including families affected by substance‑exposed newborns.
  • Experience in case management and care coordination.
Key Responsibilities

The Care Coordinator will:

  • Provide comprehensive care coordination to address each family's unique needs, including substance use treatment, mental health services, medical care, housing, financial stability, parenting support, and overall wellness.
  • Develop collaborative relationships with community partners, treatment providers, healthcare professionals, and child welfare agencies.
  • Advocate for families and assist them in accessing services that support recovery, family preservation, and long‑term stability.
  • Coordinate services using a family‑centered, strengths‑based approach that promotes sustained recovery and positive outcomes.
Qualifications

Candidates must demonstrate:

  • A strong understanding of Recovery‑Oriented Systems of Care (ROSC) principles.
  • Knowledge of case management and care coordination best practices.
  • Excellent communication, organizational, and advocacy skills.
  • The ability to work independently while collaborating effectively with multidisciplinary teams.
  • A commitment to supporting families through a trauma‑informed, recovery‑focused approach.

Position requires a Level II Background and valid driver's license.
