Role

The MACT Team Specialist functions as a Case Manager on the MACT Team, working with medical providers to address treatment. The specialist coordinates monitors and provides direct services in a community setting for consumers and acts as a member of a clinical team to assess consumer needs. They participate in development of service plans, provide and coordinate services, advocate for consumers, and assist in crisis intervention.

This person will provide after‑hours coverage and support to all persons being served on the team.

Job Duties & Responsibilities
  • Facilitate recovery and inspire hope for consumers while supporting achievement of service plan goals.
  • Provide ongoing assessment and feedback on progress relative to service plans.
  • Meet consumers in the community in accordance with policies, procedures and clinical protocols to provide supportive counseling and other services.
  • Collaborate with prescribing clinicians, nurses and other team members to ensure timely development and implementation of individual service and crisis plans.
  • Coordinate with all parties in consumer’s treatment per policy and procedure, including inter‑ and intra‑agency communication by phone and letter.
  • Assist consumers and families in accessing additional needed services and entitlements, including identification of funding services.
  • Transport consumers to necessary services as outlined in their service plans.
  • Act as primary contact for a percentage of team consumers—maintain accurate, thorough, and current documentation, update weekly client schedule and serve as first contact for consumers and families.
  • Document all consumer events according to policies and clinical protocols.
  • Participate in daily team meetings and additional treatment or team communication meetings as required.
  • Collaborate with team members to assure compliance with ACT fidelity measures.
  • Maintain knowledge of medical areas of need such as appointment scheduling and understanding diagnoses (COPD, Diabetes, Hypertension).
  • Additional duties may be required to meet program or department needs consistent with Copa Health productivity levels.
Minimum Qualifications
  • Master’s degree in a behavioral health related field.
  • Bachelor’s degree in behavioral health related field required; two (2) years behavioral health work experience preferred.
  • Minimum 2 years experience as a Medical Assistant.
  • Alternative qualifications:
    Master’s in non‑behavioral health field with 30 semester hours behavioral health education and two (2) years behavioral health work experience.
  • Bachelor’s in non‑behavioral health field with 30 semester hours behavioral health education and four (4) years behavioral health work experience.
  • Associate’s in behavioral health field with four (4) years behavioral health work experience.
  • Associate’s in non‑behavioral health field with 30 semester hours behavioral health education and five (5) years behavioral health work experience.
  • High school diploma or equivalent with 18 credit hours post‑high school education in a related field completed within the last four years and two (2) years full‑time behavioral health experience OR four (4) years behavioral health work experience.
  • Valid Arizona Driver’s License, vehicle with minimum state insurance, and ability to meet Copa Health driving requirements.
