Job Summary:
We are seeking compassionate and experienced Direct Care Workers to join our team at Upscale Home Care in Peoria, AZ. As a Direct Care Worker, you will provide high-quality care and support to our clients, empowering them to thrive in the comfort of their own homes. Must live within driving distance to Peoria, AZ.
- Provide personalized care and assistance to clients, including but not limited to: bathing, dressing, grooming, and medication management
- Prepare meals, run errands, and assist with light housekeeping and laundry
- Escort clients to appointments and social events
- Monitor and report any changes in client condition to our clinical team
- Maintain accurate and thorough records of client care and services provided
- Collaborate with our clinical team to develop and implement individualized care plans
- Provide emotional support and companionship to clients

Requirements:- 1 year of experience in the direct care industry
- High school diploma or equivalent required
- Ability to lift up to 25 pounds and stand for long periods, if needed.
- Reliable transportation and a valid driver's license (if applicable)
