Job Summary

Utilizing the nursing process, you will play a critical role in delivering high‑quality, patient‑centered care while collaborating with fellow healthcare professionals to achieve the best outcomes.
Responsibilities- Contribute professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.
- Promote the use of evidence‑based practice through protocols and procedures.
- Develop an individualized plan of care using a collaborative approach with the patient/family and health‑care team.
- Use appropriate interventions identified in the plan of care to achieve expected patient outcomes.
- Evaluate and document patient progress toward expected outcomes/goals on an ongoing basis throughout the shift.
- Modify the plan of care based on evaluation and as needed based on changes in patient condition.
- Demonstrate technical knowledge of equipment and troubleshoot when problems arise.
- Acquire and maintain current knowledge and skills in evidence‑based nursing practice.
- Share clinical knowledge through informal teaching and in response to the learning needs of others.
- Promote patient and staff safety through appropriate use of lift equipment (as applicable).
- Obtain adequate lab specimens for testing and perform bedside lab testing and associated quality control in compliance with medical provider orders and hospital lab procedures/policies.
- Coordinate the interdisciplinary care of the patient, collaborating with team members and utilizing additional resources when necessary.
- Recognize need for assistance and utilize resources, including physician and nurse‑to‑nurse (UAT) consultation in a timely manner.
- Collaborate with the health‑care team in organizing tasks and activities for the patient’s care from admission to discharge according to the plan of care and to meet identified expected outcomes.
- Provide goal‑focused shift report.
- Advocate with physicians on the patient’s behalf when necessary.
- Utilize communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes and unit productivity.
- Assess and choose teaching strategies appropriate to the patient/family’s age‑specific needs.
- Adapt teaching strategies based on patient response, readiness to learn, and level of comprehension.
- Takes cultural, age‑specific, gender, and developmental factors into consideration when teaching patients about procedures, illness, hospital experience, etc.
- Ensures patient safety in the hospital environment by following policy/procedures and adhering to National Patient Safety Goals.
- Consult appropriate resources within the hospital to provide specialized education.
- Utilize the SVI patient education system as a resource for patients and family members.
- Actively participate in data collection, analysis, and problem solving and give input on process improvements and patient satisfaction.
- Assure that the care provided is consistent with standards of care, evidence‑based practices, national patient safety goals, core measures, and other quality initiatives.
- Work with the interdisciplinary team in identifying opportunities for improvement in patient care and patient/family satisfaction.
- Actively promote and achieve customer satisfaction (patient, family, department, physicians).
- Participate in the implementation of Performance Improvement initiatives.
- Encourage coworkers in achieving excellent customer satisfaction and serve on unit‑based councils.
- Identify practice problems on the unit.
- Implement changes in own clinical practice based on literature and unit process improvement outcomes.
- Work on assigned projects, data collection, and audits as assigned.
- Certified EKG Technician (CET) – National Health Career Association (NHA).
- Licensed Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR).
- College diploma in Nursing.
