Position Summary
The Care Manager RN administers processes to coordinate and facilitate comprehensive care for pediatric individuals enrolled in Department of Child Safety Children's Health Plan (DCS CHP). This is accomplished by assessing their needs, developing personalized care plans, and coordinating services across healthcare providers. This person serves as advocate for Pediatric DCS CHP patients, ensuring effective communication, resource utilization, and continuous monitoring of their progress, in order to promote positive outcomes and enhance overall well‑being.
Required Qualifications- 3+ years of pediatric clinical care.
- Must reside in Arizona.
- Willingness to travel up to 10% of the time within Arizona.
- Must have an active and unrestricted RN licensure in the state of Arizona.
- Strong problem solving and decision‑making skills.
- Working knowledge of medical terminology.
- Excellent digital literacy skills.
- Ability to deal tactfully with customers and community.
- Ability to handle sensitive information ethically and responsibly.
- Ability to consider the relative costs and benefits of potential actions, in order to choose the most appropriate option.
- Ability to function in clinical settings with diverse cultural dynamics of clinical staff and patients.
- Bachelor’s degree, specialized training, and/or relevant professional qualification.
- Education:
Associate’s degree in Nursing.

Pay RangeThe typical pay range for this role is: $60,522.00 - $. The pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
