Position SummaryThe Community Health Worker (CHW) provides fair, accurate, and impartial information and provides enrollment assistance activities for eligible health center patients and service area residents into affordable health insurance coverage, in accordance with the Heartland Health Services’ (HHS) mission, strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards.
Essential Functions- Conducts public education activities to raise awareness about insurance coverages.
- Provides information and services in a fair, accurate, and impartial manner. Such information must acknowledge other health programs (such as Medicaid and the Children’s Health Insurance Program (CHIP).
- Facilitates selection of a qualified health plan.
- Provides referrals to any applicable office of health insurance consumer assistance or health insurance ombudsman established under Section 2793 of the Public Health Service Act, or any other appropriate state agency or agencies, for any enrollee with a grievance, complaint, or question regarding their health plan, coverage, or a determination under such plan or coverage.
- Provides information in a manner that is culturally and linguistically appropriate to the needs of the population being served by the Exchange, including individuals with limited English proficiency, and ensure accessibility and usability of Navigator tools, such as fact sheets, and functions for individuals with disabilities in accordance with the Americans with Disabilities Act and Section 504 of the Rehabilitation Act.
- Educates individuals and communities about how to use and operate within social services systems
- Educates individuals about community perspectives and cultural norms.
- Builds health literacy program(s) and cross-cultural communication(s).
- Assists in the planning, marketing, and support of special events.
- Ensures participation in appropriate marketing and outreach activities.
- Conducts health promotion and disease prevention education for participants and the community coordinating services between clinical teams and community partners.
- Participates and advocates in care coordination of services and provides referrals and follow-up.
- Facilitates social determinants of health services and address barriers to resources.
- Documents and tracks individual and population data in appropriate databases.
- Documents all consumer, patient, and community encounters in EPIC and maintains confidentiality and accurate reporting.
- Continuously expands knowledge of community resources while building resource capacity.
- Complies with CHW certified training and completes all applicable federal and state enrollment training required to assist individuals and families. Continues training with CHW peers and CHW groups.
- Provides basic screening assessments to determine those eligible for additional programs or services
- Assists with completion of applications for Social Security, Manage-my-Case, Medicaid, MyChart and appeals.
- Travels to designated counties, community locations, and various locations to perform outreach, presentations, and in-services as required.
- Demonstrates and maintains expertise in eligibility and enrollment rules and procedures, the range of qualified health plan options and insurance affordability programs, to address the needs of the hard to reach, homeless, uninsured, and underinsured populations.
- Conducts outreach and in-reach through case findings/recruitment of individuals, families, and community partners through events, follow-up services, and community groups.
- Conducts home visits to provide education, assessments, and social support, as directed.
- Supports clinic compliance with all applicable federal, state, local, and HHS rules, regulations, protocols, and procedures governing the clinical provision of medical services as well as those relating to, but not limited to, workplace safety, public health, and confidentiality.
