Overview

The Program Coordinator - Stroke and Sepsis oversees the daily activities of the Stroke Program. This position collaborates with departments, physicians and staff to ensure stroke patients receive the highest quality of care possible. The coordinator works with Education to ensure all staff have appropriate stroke-related training and partners with community outreach programs to provide stroke education. Responsibilities include overseeing stroke-related meetings, coordinating data management and analysis of aggregate clinical outcomes as per GWTG and DNV performance measures for TIA/Stroke care, leading DNV preparation and survey activities, and ensuring compliance with DNV requirements for current accreditation levels.

The coordinator collaborates with the medical director of the stroke program and the Stroke Program Committee, as well as with medical directors and the leadership team, to advance the program. The coordinator is a registered nurse who collaborates with medical and hospital staff to provide effective care to all sepsis patients, driving improvements in patient safety and mortality. The coordinator collects and analyzes sepsis performance data and uses early warning systems to identify and intervene to provide evidence-based sepsis care.

The coordinator leads the inter-professional Sepsis Committee, in collaboration with Operational Leaders, and produces scorecards and data to help drive morbidity and mortality improvements.

Responsibilities
  • Analyze data and health records from stroke patients to identify educational and quality opportunities for the stroke program.
  • Lead improvement activities in both sepsis and stroke care; coordinate responses to national, state, regional, and local changes in neuroscience care delivery models.
  • Engage neuroscience leadership in financial decision-making that impacts patient outcomes; collaborate with regional providers to implement neuroscience/stroke clinical protocols at the regional level.
  • Inform neuroscience leadership of barriers to meeting regulatory standards (DNV, ISO 9001, IDPH, CMS) and support efforts to maintain Primary Stroke Center designation.
  • Serve as a liaison between nursing staff and quality nurse specialists; provide specialty knowledge for neuroscience data collection, reporting, and analysis.
  • Develop, implement, evaluate and oversee sepsis and neuroscience/stroke education across Carle, regional hospitals, and the community.
  • Collaborate with stakeholders to align leading practices across the system and lead meetings within the stroke program (including agenda and minutes).
  • Provide staff education or inservice sessions; attend provider meetings as assigned to deliver education and current data.
  • Act as an active member of the sepsis committee and support patient care tasks during high census as needed.
  • Coordinate the organization’s response to changes in sepsis and neuroscience care delivery models during healthcare reform, engaging leadership in financial decisions that affect outcomes.
  • Lead Carle to maintain Primary Stroke Center designation and support system stroke and sepsis needs; drive process changes to improve clinical outcomes.
Qualifications
  • Certifications:

    Basic Life Support (BLS) within 30 days – American Heart Association (AHA).
  • Licensed Registered Professional Nurse (RN) – Illinois IDFPR.
  • IHI Open School Basic Certificate in Quality and Safety within 2 years – Institute for Healthcare Improvement (IHI).
  • Education:

    Bachelor's Degree in Nursing (Required).
  • Work Experience:

    Related field.

Compensation And Benefits

The compensation range for this position is $35.46 per hour – $60.99 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. Actual compensation offered will depend on factors including experience, qualifications, location, licenses, shifts worked, and compensation model.
