Pay range for this position is $31.67 - $47.47/hour. Actual pay is based on years of licensure. This is a Salaried position.

POSITION SUMMARY: The Diabetes Educator has specific responsibilities in the evaluation of each patient with diabetes. The educator must be skilled in the teaching-learning process and have an extensive background and understanding of diabetes, including diabetes management and complications. The educator has the responsibility, and must have the capability to evaluate the effectiveness of the teaching and learning that has been provided.
The Diabetes Educator protects, promotes and restores health and wellness of the patient with diabetes by practicing at the highest level of licensure; collaborating with providers and multidisciplinary care team members for provide care coordination to the patient. The Diabetes educators are members of a multidisciplinary team who cares for, educates and motivates patients with diabetes to live a healthy lifestyle.
REQUIRED QUALIFICATIONS:
- Education:
Associate's Degree or Nursing Diploma - License/
Certifications:
Current licensure by the State Board of Nursing in Illinois;
Certified Diabetes Care and Education Specialist (CDCES)–when the Mission Partner meets all of the qualifications set by the American Diabetes Association for the CDCES within 2 years of hire date or prorated by their hours worked - Other skills/knowledge:
Excellent interpersonal and communication skills. - Solid computer skills, including proficiency with Microsoft software.
- Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS:
- Education:
Bachelor’s Degree in Nursing - Experience:
6 months experience as an RN in a healthcare environment - Basic knowledge of medical terminology, ICD-10 and CPT codes Electronic Medical Record and Microsoft application basic proficiency
- BLS
