Summary:
The Lifestyle Enrichment Coordinator upholds the philosophy, vision, and core values of the organization and ensures the Christian Mission of Ever True is realized so that “Older Adults Live Life to the Fullest” by providing coordination and direction of the day-to-day Lifestyle Enrichment functions that support the domains of wellness including social, intellectual, physical and spiritual for a level of community living (IL, AL/RCF, Care Center).
Essential Duties and Responsibilities:
1. Under the direction of the Lifestyle Enrichment Director, plans, develops, organizes and implements functions to support quality of life, that enrich the lives of residents with a person-centered focus and which support domains of wellness including social, intellectual, physical and spiritual
2. Communicates with residents, their family members and other staff to learn the personal preferences, interests, needs and any limitations of the resident related to lifestyle enrichment
3. Collaborates with the Lifestyle Enrichment Director and the Lifestyle Enrichment team to develop and plan the Lifestyle Enrichment calendar;
Works with the care center or assisted living staff to enlist their suggestions for specific activities or programs which they may help conduct;
Distributes calendars to residents, families and staff and promotes lifestyle enrichment which support the domains of wellness
4. Continually researches community and online resources and keeps abreast of new ideas for planning and developing activities that support lifestyle enrichment;
Assesses resident attendance and engagement level, as well as individual resident outcomes, to help determine future offerings
5. May provide information for the electronic medical record (EMR) for Care Center residents and REACH guests including assessments, MDS, care plan information, progress notes and participation records in accordance with community, state, and federal regulations
6. May participate in care plans (SNF) and service plans (AL/RCF) as scheduled
7. If properly licensed, may serve as a back-up for the community Chauffeurs and transport residents offsite in a vehicle they are licensed to drive (DOT certification and a Class C, A, or B driver’s license if transporting 16 or more passengers including the driver;
Class D Illinois or Class E Missouri driver’s license if transporting 15 or fewer passengers including the driver)8. May supervise other Lifestyle Enrichment staff as designated
Qualifications, Knowledge, Skills & Abilities:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
1 - 3 years’ experience in an activities / lifestyle enrichment role
Must be CPR certified
Ability to demonstrate a high degree of creativity and resourcefulness
Ability to demonstrate a high level of customer service and values person-centered care Ability to demonstrate organizational skills and the ability to be part of a team Proficient computer skills
High school graduate or GEDActive driver’s license preferred
