Pastoral Bereavement Counselor – Hospice

• Provide spiritual support and counseling to hospice clients and caregivers
• Coordinate spiritual care activities with the interdisciplinary team
• Perform initial spiritual assessments and reassessments
• Develop and implement client/caregiver plans of care
• Address faith, grief and loss, prayer, illness, theology, healing and brokenness
• Contact clergy and arrange requested sacraments, including communion, baptism, reconciliation and anointing
• Provide hospice, faith-resource, coping, grief, funeral-planning and bereavement-program education
• Develop and coordinate bereavement support, assessments, plans of care and counseling
• Coordinate and conduct Loss and Grief Support Group meetings
• Plan and conduct Hospice Memorial Services twice yearly
• Coordinate the annual Camp Courageous grief camp for children
• Train and supervise bereavement volunteers
• Participate in staff orientation, quality improvement, committees, task teams and agency/health events
• Facilitate collaboration among healthcare teams, clients, caregivers, ministers and clergy
• Participate in family conferences and interdisciplinary care planning
• Provide education to staff, clergy and community groups about spiritual concerns related to terminal illness and loss
• Ensure quality and safe delivery of care
• Maintain professional knowledge and relationships with area ministers and priests

• Master's Degree in Theology, a related field, Counseling, or Sociology
• Driver's License issued by the Illinois Secretary of State
• Proof of auto insurance
• Work experience in spiritual care
• Ability to provide spiritual care consistent with clients' and caregivers' beliefs
• Knowledge of diverse religious backgrounds and developments in ministry and hospice
• Current knowledge of faith resources, spiritual care, bereavement, loss and grief
• Ability to participate in quality improvement activities and interdisciplinary teamwork
• Ability to work Monday-Friday, 8:00 a.m.
  
  -4:30 p.m., with some weekend special events
