• Support members both in‑person and virtually with real‑time consultation, education, scheduling of specialist & diagnostic referral appointments, and follow‑through and follow‑up of those appointments.
• Play a critical role in the achievement of improved health outcomes and cost savings.
• Be an empathetic problem‑solver and the most visible partner to all patients.
• Clarify and explain referral recommendations for specialist and diagnostic services.
• Manage end‑to‑end referral processes as needed.
• Lead the development and management of quality programs, including patient outreach, to elevate the primary care experience.
• Support members with chronic conditions through education, reminders, and other support.
• Handle the management and integration of office processes.
• Lead service and schedule requests on members’ behalf.
• Inform and equip the Care Team with referral information and requirements.
• Act as an ambassador in the local community by building relationships with other healthcare providers, social and community organizations, and marketing partners.
• Perform all other duties as assigned.
• Maintain confidentiality at all times.

• Current state license to practice as a Registered Nurse.
• Associate’s degree in Nursing.
• BLS certification.
• Experience with Electronic Medicare Records.
• Customer service orientation and experience.
• Strong communication skills across multiple modalities.
• Highly collaborative and proactive.
• Empathy and advocacy for patients.
• Exceptional organizing skills and attention to detail.

• Bachelor’s degree in Nursing (BSN).
• Case Management Certification (CCM).
• Proficiency with other health‑care related software platforms.
• Strong independence, flexibility, initiative, and commitment.
• Active AZ fingerprint clearance card.
