, as a Nurse Navigator at our DCAM clinic. Here, you will work alongside our Neurosciences team. You will need to be based in the greater Chicagoland area. At UChicago Medicine, we have assembled a team of neurological experts who are highly skilled in evaluating and treating the most complex and challenging nervous system disorders – with and without surgery.
Under the direction of a faculty sponsor and in collaboration with the program support team, you will coordinate the care of patients through the disease process. You will act as a clinical liaison for patients across medical services and clinical departments, assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs, and evaluate and manage patient symptoms before, during, and after treatment.
EssentialJob Functions
- Provides care coordination & patient education to patients in the neurosciences service line.
- Facilitates resolution of patient care gaps and monitors follow up/completion by proactively reaching out to patients and addressing direct phone calls and messages from patients.
- Acts as liaison for patients, families, and their multidisciplinary clinical team.
- Educates and reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on the multidisciplinary care plan.
- Develops and/or coordinates patient education materials, clinical pathways, and quality studies.
- May be responsible for direct patient care.
- Reviews new patient intake information and determines appropriate scheduling pathway.
- Performs other duties assigned.
- Registered Nurse with a current Illinois license.
- Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching, and management of patients.
- Must have general computer skills and be adaptable to changing working situations and work assignments.
- Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians.
- Certified Nurse in Specialty area preferred or in process.
