Job Overview
The Care Team, partner of Traditions Health, is seeking a new 2nd Shift RN Case Manager for our Hospice team in Peoria. The RN will provide skilled nursing care to patients of all ages in the home setting, coordinate interdisciplinary care, and manage case loads during 8‑hour 2nd shifts Monday‑Friday.

- Administer skilled nursing care for home‑health clients
- Coordinate care with interdisciplinary team, patients, families, and referring agencies
- Complete initial and ongoing assessments (OASIS, other appropriate tools)
- Re‑evaluate patient needs, monitor outcomes, and modify care plans
- Initiate preventive and rehabilitative nursing procedures and referrals as needed
- Provide education and counseling to patients and families
- Maintain infection control and OSHA compliance
- Communicate status changes to physician and staff
- Ensure compliance with third‑party payor requirements
- Prepare clinical and progress notes timely
- Participate in in‑service programs to maintain competency
- Supervise and direct other nursing personnel and aides as required
- Represent agency philosophy and policies
- Provide on‑call services as assigned
- Schedule: Monday‑Friday 2nd Shift, eight‑hour shifts
- Education: Graduate of an accredited diploma, associate or bachelor’s nursing program
- Licensure: Current Registered Nurse license; valid driver's license
- Experience: At least one year clinical RN experience; home health experience preferred

Nursing skills per standards; interpersonal skills; current CPR - Transportation: Reliable vehicle, valid driver’s license, and auto insurance
Work in patients’ homes under various conditions; possible exposure to blood, bodily fluids, and infectious diseases. Requires flexible schedule and local travel. Exposure to adverse weather. PRN emergency call.
Physical & Mental EffortProlonged standing and walking; ability to lift up to 50 lbs and move patients; handle stressful situations; make quick decisions; coordinate psychosocial needs; require hand‑eye coordination and manual dexterity.

$77,550.00 – $91,650.00
