Hybrid position with 1 day remote and 4 in office
The OP Care Management RN provides care management and population health services to patients, primarily targeting stratified risk or highly vulnerable patients during transitions between care settings. The RN coordinates care, facilitates medication management, and manages care transitions across the continuum, creating personalized, longitudinal care plans and promoting effective partnerships among patients, families, nurses, physicians, and other health‑care disciplines. A data‑driven approach is used to manage patient populations and achieve optimal clinical outcomes.

Certifications:
Licensed Registered Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR) - Education: College Diploma in Nursing (required)
- 3+ years of nursing experience
- Communicate and coordinate across care settings within the continuum.
- Identify appropriate providers, healthcare organizations, and community services; develop and maintain positive relationships with patients, families, and interdisciplinary teams.
- Provide education on medication adherence, follow‑up appointments, and self‑care to reduce readmissions.
- Empower patients and families through education, fostering trust to use resources appropriately.
- Conduct holistic health assessments, including health risk, patient preferences, goals, health literacy, engagement, confidence, comorbidities, and social determinants of health.
- Ensure accurate documentation of clinical, financial, and other information using appropriate systems.
- Utilize data dashboards or reporting mechanisms to support performance and outcome evaluation.
- Support primary care provider as part of care coordination, chronic disease education, and cross‑continuum collaboration.
- Document within EMR to support departmental workflows and ensure accurate care planning.
The compensation range for this position is $30.39 to $52.27 per hour, depending on experience, qualifications, location, and other factors. Carle Health offers a comprehensive benefits package for team members and providers.
