Nurse Practitioner, Geriatric Nurse Practitioner
A leading infectious disease healthcare provider in the Chicagoland area is seeking an experienced Nurse Practitioner (AGNP, ANP, DNP, or FNP) for a full-time, in-person infectious disease role across multiple skilled nursing and long-term care facilities in Evanston, Skokie, Rogers Park, and Northbrook. This is a Monday–Friday, no nights, no weekends position with flexible daily start times between 8:00 AM and 5:00 PM — focused on proactive infectious disease treatment, reducing rehospitalizations, and serving as a key clinical leader across assigned north suburban Chicago facilities.
The compensation package is built for experienced specialists: $50–$60/hour base, monthly performance bonuses for exceeding visit targets, CME reimbursement up to $1,000, DEA and licensure renewal covered, malpractice included, and access to Dragon Medical Dictation, Up To Date , and Johns Hopkins clinical tools.
Key Responsibilities- Diagnose, treat, and manage infectious diseases across skilled nursing and long-term care facilities
- Perform comprehensive physical assessments and monitor changes in patient conditions
- Prescribe medications and develop individualized treatment plans
- Participate in and guide residents' plans of care
- Coordinate advanced care planning and discharge processes
- Facilitate referrals and collaborate with physicians and specialists
- Maintain a minimum of 12 patient visits per day (bonus eligible for exceeding targets)
- Ensure timely, accurate documentation and communication across facilities
- MSN from accredited NP program required
- 1–3 years NP experience; infectious disease experience strongly preferred
- Experience in acute, sub-acute, or long-term care settings required
- Background in geriatrics, internal medicine, primary care, hospice, or palliative care preferred
- Knowledge of Medicare, Medicaid, and managed care structures a plus
- Strong interpersonal skills with elderly populations
- $50–$60/hour + monthly performance bonus
