Job Description
UChicago Medicine is seeking a highly skilled and motivated Community Health Nurse to join the Live Healthy Chicago team.
As a Community Health Nurse, you will work autonomously to support patients and partner clinics in implementing quality improvement strategies that enhance hypertension control and patient outcomes across Chicagoland neighborhoods.
Essential Job Functions- Partner with primary care clinics and community organizations to lead and support quality improvement initiatives focused on hypertension management.
- Deliver education to patients with hypertension in community and clinical settings, promoting self‑management and healthy lifestyle changes.
- Collect, analyze, and report clinical and quality data to drive continuous improvement efforts.
- Act as a clinical liaison among patients, providers, and community stakeholders to improve care coordination and engagement.
- Conduct outreach activities to increase program participation and awareness in target communities.
- Support CHW efforts through both education and triage of community participants.
- Ensure compliance with requirements and maintain thorough documentation.
- Collaborate with interdisciplinary teams to support program planning and sustainability.
- Current, unrestricted Registered Nurse (RN) license in Illinois.
- Minimum 3‑5 years of nursing experience in community health, public health, or ambulatory care, with a focus on chronic disease management preferred.
- Proven ability to work independently with strong clinical judgment, organizational skills, and communication abilities.
- Experience with quality improvement processes and data‑driven clinical practice.
- Cultural competency and experience serving diverse, underserved populations.
- Proficiency with electronic health records (EHR) and data management systems.
- Willingness to travel within Chicago for community outreach and clinic support.
- Commitment to advancing health equity and improving outcomes for vulnerable populations.
- Preferred certification in Public Health Nursing or related specialty.
- Experience working in community‑based health programs.

The pay range reflects the anticipated wage or salary reasonably expected to be offered for the position, based on a full‑time equivalent (1.0 FTE) and reviewed annually. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
